Facility Request Form

To Schedule an Event at FBBC, to Reserve a Room, or to Request Transportation:

Approved events are placed on the church calendar when a Facility Request Form is completed and submitted to Sandra Henson in the Church Office. This downloadable form below may be completed and returned to Sandra via email or via fax at (225) 272-0151.

Due to the number of events at FBBC and the fact three different congregations worship at this location, completed forms should be submitted at least two (2) weeks prior to the event. If you have questions about the form, please contact Sandra at (225) 272-3740, ext. 312.
